This is the wording the British embassy specified during my step daughter's visa application. They had previously rejected the first one issued by the amphur.
Interestingly (is there such a word ?) the Amphur held on to the wording for future use I presume.

My wife has read the Por Khor 14 Example above, as said this is for someone that was married. My question is does my wife still need a Por Khor 14 for her 10 year old son's visa application, as her name is the only one that appears on his birth certificate.
