  • AN Form - A Few Questions

    I've just started filling out the AN form for my Wife and there is a question which is puzzling me and it could be because I am thinking too hard!

    First Question -
    Section 1: Personal Information.
    Question 1.7 - Name at birth if different from above. (If the names you have given are different or spelt differently from the name shown on your passport, please explain why on page 14)

    I assume we have to put my Wife's maiden name in here and explain this on page 14.

    Am I right on this?

    BTW Her Thai passport has her married name in it and NOT her Maiden name.

    Second Question -

    In Section 1.17 Please provide your addresses for the past five years.
    My wife first came to the UK on a Visitors Visa then went back, then we got married in Thailand and she came here on a settlement visa and now an ILR visa.

    Do I just complete this section with the addresses for both UK and Thailand that covers five years?
